New Patient Registration
Patients will be asked to complete registration forms and, based on the information given, may be invited to attend a new patient check with one of our nurses. This enables us to take a full medical history and we are able to inform patients about the various services we offer. You may also be asked for details like next of kin, whether you are a carer for someone and which nurseries/schools any children may attend.
Patients requesting to register from other Maidenhead practices - due to the volume of 'internal transfers' we are receiving we have stopped automatically registering patients from other local practices without there being a good reason to do so. If you are applying to register from another Maidenhead practice then we ask that you include a cover letter explaining why you are looking to change practices for review by the Partners. With the current high demand on appointments, it is important that we are able to provide access to our existing patients and those new to the area living within our catchment area.
Patients will be allocated a named GP, but are free to consult with any of our healthcare professionals.
If you have guests staying with you who are normally resident in the UK, we can see them as temporary residents if they should fall ill. Forms are available at reception. Youngsters home from university/college can be seen as temporary residents, but their routine care should continue to be provided by their usual GP (this includes repeat prescription requests, etc...).
The Cedars Surgery operates an inner and an outer catchment area. New patients to the surgery must live within the inner boundary, however, registered patients moving from the inner boundary to within the outer boundary will not be required to re-register.
